Financial Aid Forms

The following are financial aid forms for the 2025-26 academic year (summer 2025, fall 2025, spring 2026):

1. Enrollment Worksheet - This form is for students to notify the BW Financial Aid Office if there is a difference between their actual registered credit hours and the anticipated enrollment reflected on the financial aid offer.
2. Special Circumstance Review - This form should be completed by students and their families if they're experiencing a financial situation that is not reflected on the student's 2025-26 FAFSA.
3. Verification Worksheet - To be completed by students who have been selected for verification, only if requested. Do not complete this form unless you have been specifically asked to do so by the BW Financial Aid Office.
4. Federal Direct Loan Change Form - This form should be completed by students who have previously accepted the Federal Direct Student Loans offered to them but wish to reduce or cancel the loan(s).
5. PLUS Denial - Additional Unsub Request Form - Students are able to request an additional Federal Direct Unsubsidized loan if their parent has applied and been denied for the Federal Parent PLUS loan.
6. Parent PLUS Loan Change Form - This form is to be completed by parents who have already applied for the Federal Parent PLUS loan but wish to change the amount. Please note: Parents must apply for this loan on a yearly basis through the Department of Education and the application will not be available until mid-April.
7. Grad PLUS Loan Change Form - This form is to be completed by graduate students who have already applied for the Federal Grad PLUS loan but wish to change the amount. Please note: Students must apply for this loan on a yearly basis through the Department of Education and the application will not be available until mid-April.
8. Transient Financial Aid Consortium Agreement - To be completed by BW students who are enrolled at least half-time at BW and will also be enrolled at another institution. Be sure to submit all required documentation with this form. Requests cannot be processed until all documents have been received.
9. Scholarship and Special Award Application - This application can be completed by students to determine if they're eligible for any endowed scholarships.
10. BW Employee Tuition Remission Application - This application is to be completed by employees of the University who are requesting tuition benefits at BW. The HR form can be accessed in Forms Central. Employees should navigate to the Campus Applications section in Jacket Connect.
11. TEACH Grant Request Form - This form is for students interested in receiving a TEACH grant. Visit BW TEACH Grant Policy for details on eligibility.
12. IRA Rollover Confirmation Form - This form should only be completed if requested.
13. Grade Level Advance Loan Request - This form is for students who have advanced a grade level after the fall 2025 semester and would like the BW Financial Aid Office to process their new federal student loan eligibility.
14. SAP Appeal Form - This form is for students who have been notified that they're not meeting Satisfactory Academic Progress (SAP) and need to appeal for financial aid reinstatement.
15. Financial Aid Submit Supporting Docs Form - This form should be used to attach any supporting documentation that you would like to submit to the Financial Aid Office.
16. Financial Aid Housing Change Form - This form notifies BW's financial aid office of your completed change in housing status. You will need to complete a housing change form with residence life prior to completing this document. If you are looking for an estimate of that change, please contact the BW Financial Aid Office at finaid@achador.net with your BW Student ID number and your request for a housing change estimate.
